A statement, under penalty of perjury, of your good faith belief that the removal was a mistake.
Your contact details and a statement of consent to local jurisdiction.

4. Repeat Infringers
Epix IPTV reserves the right to terminate the accounts of users or restrict access for those determined to be repeat infringers of intellectual property rights.
5. Misrepresentation
Please note that under Section 512(f) of the DMCA, any person who knowingly misrepresents that material is infringing may be subject to liability for damages.
6. Changes to this Policy
We may update this DMCA Policy at any time. Changes are effective immediately upon posting on this page.
